Migraines, irritable bowel syndrome, essential hypertension, insomnia, chronic anxiety are just a few of the ongoing complaints of domestic abuse survivors. And it s not surprising. These conditions are ALL mediated by the sympathetic nervous system-a system perpetually “on” for individuals whose lives are entangled in physical, emotional and verbal abuse. Stemming back to our caveman ancestors, we inherited a physiology that is built to protect us from the perceived threat of impending danger. And when present, sympathetic arousal occurs. For example, when the caveman saw a tiger, the thought “tiger” registered, activating a limbic system response of “fear”…ultimately, resulting in an adrenergic response that prepares the body for “fight or flight.” And with this, of course, mobilizing the proper body systems to deal with the circumstances at hand. Here s what happens: blood rushes to the core of the body and away from the periphery, fueling the core organs, (heart and lungs) for this magnificent maneuver. Blood is pumped to the large muscle groups to enable rapid exit or fierce fight. Numerous other physiological changes occur. You re getting the picture, correct? In a nutshell… 1) Your cardiovascular system is aroused: increased heart rate, rise in blood pressure. 2) Your neuromuscular system is activated: increased muscle tension. 3) And your gastrointestinal system shuts off: dry mouth, build up of acidity in the gut, bowel and bladder relax. This is sympathetic arousal; that is the activation of the sympathetic nervous system. It is our body s response to impending danger. It s your survival mechanism, your natural stress response. This is good. However, here s where it s not. In the same way that our bodies are built to trigger this response, so are we built to cycle down when the impeding danger or perceived threat passes. And since our physiology can t distinguish the difference between real and perceived threat, we have the ability to keep this stress response “on” such that complete cycling down fails to occur. Then when the next tiger comes our way, naturally we are appropriately aroused, but this time the excitation starts from an already elevated point of arousal, thereby resulting in a higher-level stress response. Now for a moment, let s envision our creating a habit, if you will, of not fully cycling down between each perceived threat-between each tiger crossing our path. What happens? Your body systems associated with the stress response remain “on” and at higher and higher levels of arousal over time with subsequent activation. This is what we call “chronic stress.” Here s the danger. When you keep these organ systems functioning as such eventually they show symptoms of excess activation. This is the basic etiology of stress related illnesses: migraine headaches, muscle contraction headaches, irritable bowel syndrome, essential hypertension, insomnia, chronic anxiety. Eating Out - What Can You Order if You are Salt-Sensitive? I became salt-sensitive when I reached middle-age. When I eat too much salt my feet, knees, and eyes swell. Sometimes I have “pillows” under my eyes and I reported these symptoms to my primary care physician. She prescribed a diuretic and I take half a pill a day. The medication helps, but I still have to be careful about how much salt I eat. According to “Decreasing Your Salt Intake,” an article by Karen Schroeder, MS, RD, published on the Emory HealthCare Web site, salt sensitivity is difficult to diagnose. Some salt-sensitive people have high blood pressure (hypertension). “Nutrition researchers are still trying tease out the exact role of sodium in hypertension,” notes Schroeder. I do not have hypertension, yet I am salt-sensitive. Eating out is hard. WebMD published an article about eating less salt, “5 Tips for Reducing Salt Intake.” Restaurant meals are a source of salt, the article says, along with condiments (ketchup, soy sauce, Worcestershire sauce), processed meats, and snack foods. Before I order anything I speed read the menu. Since appetizers tend to be high in salt I seldom order them. However, I have ordered a healthy appetizer instead of dinner. What else do I order? FRESH PRODUCE. Fresh fruit and vegetables are the first things I look for on the menu. If I order a salad I ask for dressing on the side. A LITTLE BREAD. Specialty breads like garlic toast contain lots of salt. I love bread, but monitor how much I eat. Besides, I do not want to fill up on bread before my dinner comes. PLAIN MEAT. Grilled meat and fish are my first choices, but many restaurants soak their meat in high-salt marinades. I ask the server if the meat has been marinated. Thank goodness restaurants like the Outback Steak House will let you order a plain steak. The chef will gladly prepare your chicken, fish, and vegetables without salt. SAUCE-FREE ENTREES. The chef may use commercial stock to make sauces. These products are loaded with salt, so I look for entrees that do not have sauce or gravy. I would never order stew in a restaurant. WATER. Regular soda pop and diet pop contain more salt than you may think. A 12-ounce bottle of “low sodium” Sprite, for example, contains 70 milligrams of salt. Since I do not need extra salt and want to protect my tooth enamel I always order water. I may also order iced tea and coffee. DOWNPLAY DESSERT. Usually I am so full after a meal that I have no desire for dessert. A scoop of ice cream may tempt me, but that is rare. Restaurant portions have gotten huge. I ask for a box and take half of my dinner home. These leftovers make one or two lunches. Salt-sensitive people can eat out, but you have to be alert and order wisely. Plant sterols have cellular functions in plants similar to cholesterol in humans. More than 40 plant sterols have been identified, but sitosterol, campesterol, and stigmasterol are the most abundant. Stanols are saturated sterols. The major plant stanols are sitostanol and campestanol; they are much less abundant in nature than sterols. Foods with plant stanols or sterols added, for example some margarines, can lower blood cholesterol levels by reducing the absorption of cholesterol in the intestines. Most people consume 200-300 mg of plant sterols and stanols through eating vegetables and fruits as part of their normal daily diet. Studies prove that it takes at least 1.8 - 2 grams a day to obtain the cholesterol lowering effect. This fact is based on more than 40 years of research. The average consumption of spreads (margarine and butter) in the United States is between 10-15 g per person per day. If you increase this level to 25 g per day of margarine enriched with plant stanols and/or sterols, the 2 gram amount suggested above is obtained. By just replacing butter with ordinary polyunsaturated margarine a person can lower blood LDL bad cholesterol concentrations by about 12 mg/dL. But the expected reduction from margarines enriched with plant stanols and/or sterols is about twice as high - as much as 27 mg/dL. This represents a reduction of one quarter in the risk of heart disease. Margarines containing either plant stanols or sterols are marketed in many countries: Benecol and Take Control are two brands here in the United States. Also yogurt, cream cheese spreads, and cereal bars have been introduced in some countries. And enriched cereals and fruit juice are being test-marketed in the United States. To determine if the product is enriched, look at the label on the product under the Ingredients section (listed following Nutrition Facts ). Studies are ongoing, but so far the consumption of these enriched foods has not produced any adverse effects. It has been suggested that these nutrients may interfere with the absorption of the oil-soluble vitamins (A,D,E,K). If this is true most of the vitamin deficiency could be made up by eating a few more fruits and vegetables. As an extra bonus - it has been shown that patients treated with cholesterol-lowering drugs obtain additional significant LDL reduction when these plant nutrients are added to their diets, enabling many of those not at their LDL goals to achieve those goals. But it s best to consult with your doctor before you attempt this. The consumption of foods enriched with sterols and/or stanols should complement a healthy diet low in saturated fat and cholesterol and high in fruits, vegetables, and whole grains, with unhardened oils as the fat source. The latest on the benefits of beetroot are causing excitement around the world. It s arrived at number one, top of the high blood pressure foods, after being discovered by researchers in the UK, that drinking 500ml of beetroot juice every day may reduce high blood pressure significantly. The benefits of beetroot consumption was clear to see when the drop in blood pressure reached it`s peak 4 - 5 hours after chewing and swallowing, and continued to show some reduction 24 hours later. Professor Amrita Ahluwalia, the leading researcher, had this to say, “Our research suggests that drinking beetroot juice, or consuming other nitrate-rich vegetables, might be a simple way to maintain a healthy cardiovascular system, and might also be an additional approach that one could take in the modern day battle against rising blood pressure.” Whether or not further research will reveal if this will work on hypertensive patients, is yet to be seen. A level of 200 to 239 mg/dl indicates a moderate risk; one that exceeds 239 mg/dl indicates a serious risk of CAD. High-density lipoprotein (HDL) and LDL cholesterollevels may help predict the risk of CAD more accurately than total cholesterol levels. An elevated LDL cholesterol level indicates an increased risk of CAD, but a high HDL cholesterol level indicates a lower risk. A series of cardiac serum enzyme assays can confirm an MI. Total creatine kinase (CK) levels rise within 6 hours after the start of an Ml and peak in 12 to 24 hours after cardiac tissue death. When cardiac tissue dies, CK-MB isoenzymes, which are found only in myocardial cells, enter the bloodstream. Measuring their level can help determine the amount of myocardial damage. A Day in the Life of a Buddhist Forest Monk (Afternoon) It s four p.m. I have been napping and doing walking meditation since the morning meal. My hut is deep in the forest, situated on the upper end of a massive, flat rock, with large, flat rocks on both sides and deep ravines separating them (havens for cobras). Surrounding everything is dense jungle. The six by seven foot hut is perched on the customary four stilts, with each stilt fitted with a small pan filled with kerosene to keep out ants and termites. Eight steps lead to a small porch at the entrance of the small hut, which has two large windows with shutters to protect me from the heavy storms that would soon arrive. It has remained untouched by the fierce lightening so far. The tin roof holds up well during the rains and is clear of low hanging branches that would invite vipers to drop off trees and become unwelcome guests. Inside on the floor is my lantern and a water jug, and in a corner is a table with a candle and some incense. The solitary adornments on the back wall are a pair of geckos, the ever-present foot long lizards that consider this hut their home as well. The floor and walls are made of planks cut from large trees by villagers using a two-man saw. This is backbreaking, tedious work for the young village men who manually cut forty-foot logs end-to-end to make boards. They will work all day without stopping, except for a few bites of rice and a coke at noon. These impoverished villagers give up a great deal of their time and resources to support the monks, and I vow to work as hard as I could to find the truth so that I could somehow repay them. Their generosity astounds me. A monk s routine in Thailand varied little no matter where he stayed. Now, at four o clock I will carefully sweep my half-mile path to the main hall so that snakes can t hide in the leaves, and then I will join my fellow monks at the well where we each draw a bucket of cold water for our bath. This bathing area also serves as a meeting place where the monks meet twice a month to make their brooms for sweeping the paths and to wash and dye their donated robes by boiling them with the orange bark from the jackfruit tree (and thus we were treated to a hot bath every two weeks!). There might be a cremation in the afternoon. It s 1981, and Thai families lose as many as half their malnourished children to diseases such as malaria, typhoid fever, dengue fever, hepatitis, rabies, dysentery, cholera, malaria, hepatitis, Japanese Encephalitis, and snakebites. The cremation fires remain busy. The first cremation I witnessed involved a small girl, six years old perhaps, so beautiful, her long, black hair combed so carefully with a pink ribbon tied on the side. She looked as if she were only sleeping. I vividly recall the fire becoming extremely hot once the branches were lit, and in only moments, her shiny black hair sizzled, and then was gone. Next, the skin on her face blistered, and was gone as well, exposing the white skull underneath. The little body blackened quickly, its limbs curling up into a fetal position, and then it began cooking. The dramatic memory of the episode remained with me for weeks, as the monks warned it would, and it was some time before the skulls that appeared on my kuti wall every evening in the candlelight, departed. In those days, the cremation pits consisted of nothing more than four long stakes pounded into the ground with the space between filled with stacks of dry limbs and twigs. The parents would place the body of their child on top of the heap, after which they would stand stoically by to watch it burn. The mother would throw candy into the air, and the father, sitting on his heels, would smoke cigarettes. Expressing emotion was not considered appropriate etiquette by the Thais, and yet at times I caught glimpses of mothers off by themselves crying quietly. It wasn t considered proper to make a spectacle of yourself. The cremation ceremony is over, and I return to my hut now.
